Kansas Health Foundation Grant Character and Leadership Survey

The Boys and Girls Club has received a grant to purchase program materials for the club. The grant requires we survey members to know how they feel about some things covered in the program and how they might act at times. Below is a set of items about what people believe and how people act. Please read each item carefully and then click on the button to the right to show how much you agree or disagree the statement sounds like you. If you cannot decide about the statement, click on “Undecided”. There are no right or wrong answers. No one at the club will know how you marked the statements. Please be honest in your answers.
