ASCD Self Rating Grid

The funding source for the R3 TEACH grant requires an annual grant evaluation.  Your response to the items below will be very helpful to evaluators in assessing the impact of current grant activities on program participants. While collective group data will be shared with the program planners, your individual responses will be seen only by the evaluators and will remain confidential.

Filling in your name and  MYWSUID  is necessary to verify that a survey has been received from each R3 TEACH teacher candidate. 

Begin the survey by identifying the year you started in the R3 TEACH  program. Across the top of the grid are five levels of professional practice. The first column describes the four program domains. After reading the descriptors in the leftmost column, click on the button to the right that best describes your overall level of practice for EACH DOMAIN. The format requires you to choose a performance level for each domain.  Please complete the survey by May 1, 2010

NON-USE Have not yet learned about the components.

KNOW (Awareness) Am aware of the basic components, principles, and terminology.

SHOW (Demonstration) Have used the basic components occasionally. Seek further knowledge about the components.

GROW (Integration) Frequently practice the components. Feel confident applying the components throughout the curriculum when appropriate.

GO (Transfer) Use the components consistently. Assist colleagues in applying components to their specific teaching assignments.
